Community Transformers Equity and Impact Innovators addresses the diverse challenges faced by communities in our healthcare system; it is crucial that we focus on advancing health equity. Health disparities, whether related to race, socioeconomic status, geography, or other factors, create barriers that prevent many individuals from achieving optimal health outcomes. 

 

Achieving health equity means ensuring that everyone has a fair and just opportunity to be as healthy as possible. This requires not only addressing disparities in healthcare access, but also in social determinants of health such as education, housing, employment, and access to nutritious food. We can start by:

We Are Visionaries

  • Promoting Inclusive Healthcare Policies: Implementing policies that prioritize vulnerable populations and remove systemic barriers to care.

  • Expanding Access: Ensuring that healthcare services are available and affordable to everyone, regardless of where they live or their income level.

  • Community Engagement: Actively listening to and collaborating with communities to identify unique needs and solutions that work for them.

By focusing on these priorities, we can begin to dismantle the barriers that perpetuate health inequities and ensure that every individual has the resources and support needed to achieve their best health.

OUR MISSION

CTEII promotes collective work and responsibility within the communities we serve. Our staff consist of some of the most respected professionals in the area who possess the knowledge and experience to build  coalitions that will greatly benefit the community and those who live within. CTEII’s mission is to be an organization that is dedicated to building collaborations within communities to develop sustainable programs that address the social determinants of health and promote health equity.

Our vision is deeply rooted in the belief that all people deserve a healthy, productive, and prosperous existence, and that in order for a community to reach its full potential, it must provide  the necessary tools for its people so that they can  prosper,  assume healthy lifestyles, and  enjoy a sense of  self-appreciation and social equity.  The Community Transformers Foundation is committed to the communities it served and it is our mission to ensure that people have the resources they need to succeed.

OUR VALUES

A healthy community is crucial to achieving successful outcomes, whether social or economic. The Community Transformers Foundation addresses health gaps in communities where access to quality healthcare is limited, and connects communities with a network of healthcare professionals who can provide screenings, counseling, and education so that people will have the information they need to make the best decisions regarding their health and wellness needs

OUR STRATEGY

CTEII’s strategy is to partner with community leaders, community-based organizations and residents to develop sustainable programs that will ameliorate issues around health inequity and social determinates of health.  CTEII's board of directors were chosen specifically for their knowledge, relationships to leaders (either as one or connection to) and their passion for empowering communities. CTEII builds collaborations with the communities they serve to design and implement programs to promote healthy outcomes.

MOMMY CONNECT

Mommy connect is a program for mothers of all ages who battle with depression, anxiety and the stresses of single parenthood. This program is in place to support those women in marginalized communities in coming together to figure out ways to cope and overcome these obstacles. This support group is broken down into 8 weeks of virtual behavioral health led sessions. Each session provides women with different coping mechanisms, local and/or national resources along with educating them on the importance of  healthier lifestyle changes for both mom and baby. The goal of Mommy Connect is to promote mental health awareness and create a sisterhood that will inspire and empower women by letting them know they are

never alone. Mommy Connect has supported women in the past and has helped them to become healthier mothers and parents for their kids. For more information email info@cmtytransformers.org.
